Upon your arrival please be prepared to provide the Registration/Admitting Office with the following:

• Name (photo identification is required)
• Address
• Date of Birth
• Social Security Number
• Emergency Contact Information
• Insurance Card and Information (Confirm your insurance coverage by contacting your provider to verify all aspects of your coverage.)
• Prior approval or pre-authorization form
• We may collect a portion of your insurance deductible and/or co-payment at the time of registration.

You may also need to bring:

• Credit card or check book.  We discourage you from bringing cash.
• Test results from other facilities
• Advance Directives

If you are being admitted to the hospital you will be given an identification wristband that will ensure you are given the proper care and treatment during your stay.  You will need to leave it on throughout your entire stay.
