Your Florence Healthcare team will decide when you are ready to be discharged.  You or an assigned caregiver will help you know what care you will need once you get home.  Please plan to have transportation or if you need assistance, medical transportation can be arranged.  Discharge instructions will contain all necessary information to make follow-up appointments as well as instructions on medication.  If you have any questions or concerns about your discharge instructions or follow-up treatment, please discuss with your physician or nurse before leaving Florence Community Healthcare.
